The project aims to use multidimensional prognostic indices(MPI)that predict survival, based on a Comprehensive Geriatric Assessment (CGA), to improve the cost-effectiveness of health interventions in older individuals with multimorbidity and polypharmacy. The best setting-specific MPI-profile in which individual interventions are effective and reference models for allocate resources in healthcare will be identified to promote integrated care pathways of interventions.
The general objective is to reduce unnecessary use of health care resources according to appropriate and tailored, integrated multi-professional, planned interventions to the older subjects.

The project ICARE4EU aims to contribute to the innovation of care for European citizens with multiple chronic conditions by increasing and disseminating knowledge of potentially effective and efficient models of integrated care for people with multiple chronic conditions that are developed and implemented in (parts of) European countries. The vast majority of people with multimorbidity are older individuals. Integrated care addresses their integral needs for health and social care, and is provided by multidisciplinary teams of professionals and/or informal carers.
ICARE4EU will describe the state of the art in 30 European countries; evaluate the strengths and weaknesses of current integrated care programmes; identify best practices from different perspectives; generate knowledge about key characteristics and conditions for successful management and implementation of integrated care programmes; design a template for future monitoring of the developments in multimorbidity chronic illness care; and disseminate its findings throughout Europe.

Etude de l’impact de la canicule à l’échelle européenne. Notre projet vise d’abord à déterminer l’ampleur de la surmortalité totale en Europe au cours de la canicule de l’été 2003 en terme de nombre de décès, en précisant les pays et les périodes concernés. Il vise ensuite à déterminer l’impact de la canicule sur la population des personnes très âgées en terme de taux de mortalité par année d’âge. Quelle fraction des plus âgés sont morts au cours de l’été 2003 ? Il vise aussi à étudier les effets tardifs de la canicule. L’excès de mortalité est-il suivi ou non d’une phase de sous mortalité ? La canicule élimine-t-elle ou non les plus fragiles ? La canicule affaiblit elle ou non les survivants à la vieille de l’hiver ? Cette étude permet de mesurer précisément l’impact des différentes températures et de leurs variations sur la mortalité des personnes âgées. Afin de pouvoir ainsi apprécier le rôle de l’environnement dans les trajectoires de mortalité que nous observons aujourd’hui aux âges extrêmes. Notre projet vise enfin à organiser une conférence européenne pour présenter et discuter les résultats de l’étude européenne ainsi que ceux des différentes études nationales en cours. Un des ‘ouputs’ de la conférence pourra être la proposition d’un système ou d’une méthode de surveillance des excès de mortalité liés aux conditions climatiques.

The aim of the project is to collect empirical, reliable and representative data on the magnitude, characteristics and determinants of abuse of elders, and its health and well-being outcomes. The project will also contribute to the development of evidence-based policies, research and practice strategies aimed at early detecting and preventing abuse, but also at intervening to support/care of the abused elder individuals.
